Physical Check-up for Three-year-old Infants.How to Test Eyesight
To prevent weak eyesight, Sakai City is conducting a At Home Eyesight Check-up for three-year-old infants. Please use the set in the envelope.
Please follow the instructions below.
Please contact your local public health center if you have any questions.
（1）First, take out the items from the envelope.
(2)Cut out the handle for child.
(3)A guardian (or tester) holds the target for practice and the child holds the handle.
(4)First, try at close distance with both eyes open. Do some practice so that the child can match the direction of the circle’s edge with that of the guardian’s one.
(5)Next, practice with the mask on and with one eye closed.
Now do the real check-up
(6)Conduct a check-up in a bright room.
Measure 2.5 meters, using the string in the envelope.
(7)A guardian holds the check-up Target (a small circle).
(8)Conduct a check-up at the child's eye level.
Refer to the image above for carrying out a check-up.
(9)When changing the direction of the circle’s edge, hide it first and do so.
(10)Begin with both eyes open.
(11)Next, with the mask on and test four directions (up, down, left, and right) with the right eye and then the left.
(12)Write down the result of the check-up in the Questionnaire on At Home Eyesight Check-up. Please make sure to answer all questions. (Questions that are often missed are underlined.)
