Vaccination Certificates of COVID-19 (Vaccine Passport)


Digital Certificates (Free of charge)

Digital vaccination certificates (for both domestic use and international travel) are available and it is possible to apply via smartphone app and display digital certificates with 2D barcodes on screen.
※Vaccination completion proof (接種済証) or vaccination record (接種記録証) given upon inoculation is still valid for domestic use.

How to obtain digital certificates

Search by “接種証明書”(sesshu shoumeisho, vaccination certificates) in the App Store or Google Play. Please see the website of the Digital Agency for more detail.

What you can do with the app

  • Obtain certificates both for domestic use and international travel
  • Display certificates anytime with app
  • Read 2D barcodes of other vaccination certificates

What you need to start using the app

  1. My Number Card (Individual number card)
  2. PIN number for My Number Card (4 digit number set when receiving the card)
  3. Passport (only for those applying for international travel)

System requirements

  • Reader terminal for My Number Card (NFC Type B compatible)
  • iOS 13.7 and later
  • Android 8.0 and later

※The 2D barcodes printed on certificates include personal information (name, date of birth, vaccination records, etc.), so please handle with care.

Paper Certificates (Free of charge)

Paper certificates for domestic use or international travel use can be applied for by post only.
※It takes at least about 2 weeks to issue certificates after receiving the application. If confirmation of vaccination record requires more time, issuance may be delayed.


Those registered as residents in Sakai City when vaccinated and plan to go overseas

Necessary items

  1. Application form
  2. Passport
  3. ID with individual number (My Number) or address, such as resident card, individual number card (My Number Card), certified copy of resident register (juminhyo) with individual number, driver’s license, health insurance card etc. ※if you use your resident card or health insurance card, a copy of bothsides may be needed.
  4. Vaccination completion slip (sesshuzumisho) or copy of preliminary examination sheet (yoshinhyo) of the second dose. It will take longer time to issue a certificate without this.
  5. Document showing the address to be mailed. (Unnecessary if confirmed with an item listed above)
  6. Return envelope with postal stamps (84 yen for regular size ordinary mail).
  • In the case of entrusting to an attorney, please also submit:
  1. Power of Attorney
  2. Attorney’s ID, such as driver’s license, health insurance card, individual number card (My Number Card)
  • In the case of address and/or name change after vaccination, please also submit
  1. Document showing the change of address and/or name, such as certified copy of resident or family register (juminhyo or koseki)
  • In the case name shown on passport and resident register are different, please also submit
  1. Document showing the person with the name on passport and the person with the name on resident register are identical, such as driver’s license, certified copy of resident or family register (juminhyo or koseki) with former-, common- or by-name.

How to apply

Please send by mail necessary items to:

※Please send copies of necessary items except for an application form.
※Usually it takes about a week to issue a certificate after receiving an application.
