About Identity Verification Requirements at Service Counters

Recently, there have been cases in which people have successfully impersonated others and made applications, notifications, or requests, and thereby received certificates by unlawful means and for illicit purposes.

In accordance with the amendments made to the Basic Resident Registration Law and the Family Registration Law on May 1, 2008 (Heisei 20), applications for the issuance of certificates or notifications regarding the family register and resident registry, require proof of your identity at the counter.

We ask for your cooperation in order to prevent false or fraudulent requests by third parties and to protect the personal information of everyone.

1. Filing Applications, Notifications, and Requests at Civic Affairs Division Service Counters

Please show the following documents which can verify your identity and answer any questions about the details to confirm your identity. In addition, please understand that the staff might make copies of your documents.

(1) Documents deemed appropriate by the mayor to prove your identity, such as a passport, driver's license, Basic Resident Register Card, or other licenses, permits or certificate of qualification issued by other public offices (limited to those with your photo on it).

(2) Other documents deemed appropriate by the mayor.

(3) If you cannot present the above documents that can verify your identity, the counter staff will ask you about the details.

(4) Even if you have shown the above documents that can verify your identity, the staff might ask you detailed questions to provide additional information when necessary, in order to handle personal information with the utmost care.
